Please complete one form for each virtual child to sign up for delivery of a meal box (five (5) breakfasts and five (5) lunches) to your residence on Wednesdays between 7 a.m. and noon. To grant permission for delivery of meals, you must complete a separate form for each 100% virtual student enrolled in St. John the Baptist Parish Public Schools in your home.
Meal boxes will be delivered by a school bus driver, once per week, on Wednesdays (excluding school holidays, closures and severe weather). The delivery person will ring the doorbell or knock. Meals cannot be left on a doorstep; therefore, someone must be there to receive it. If no one is home to receive the meals, the meal box will be returned to the school(s).
Meal boxes will include a combination of fresh bread products, cereal, frozen entrees, frozen juice, pre-portioned fruit and vegetables, and shelf-stable milk or fresh milk. All food should be refrigerated immediately if not eaten. Instructions for heating and refrigerating will be included.
To receive deliveries in February, you must complete this form by January 20, 2021.
