To All Parents/Guardians and School Staff:
This is an important reminder that all students currently enrolled under an affidavit must
reapply for the 2025-2026 school year. Affidavits are valid for one (1) school year only and
must be renewed annually.
What Is an Affidavit?
An affidavit allows a student to enroll in school when they are not living with their legal parent
or guardian, but instead reside with another adult-referred to as the caregiver.
Who Is the Parent/Guardian vs. the Caregiver?
• Parent/Guardian: The legal parent or court-appointed guardian of the student.
• Caregiver: The adult the student currently resides with who is not the legal parent or
guardian. This person is responsible for the student's daily care.
What is Provisional Custody by Mandate?
A Provisional Custody by Mandate is a temporary, notarized document that allows a parent or
legal guardian to give another adult permission to care for a child.
2025-2026 Affidavit Renewal Requirements:
To complete the renewal process, families must submit the following:
1. Copy of the caregiver's photo ID (e.g., driver's license or state-issued ID)
2. Parent/Guardian's address must match their Driver's License or State ID
3. Two (2) current proofs of residency (utility bill, lease, mortgage, etc.) from the caregiver
4. Optional but recommended: A Caregiver Authorization/Provisional Custody by Mandate
Form. This form gives the caregiver the right to make medical or academic decisions for
the student.
All documentation must be submitted to the school by July 17, 2025, to finalize enrollment
for the 2025-2026 school year. For any questions or support with the affidavit process, please
contact your child's school or the District Student Services Office.
